Quarantine Declaration

If you travel to the Netherlands from or by way of a very high-risk area, you must complete a quarantine declaration form before entering the country.

  • Even if you fall into one of the exemption categories (e.g. fully vaccinated);

  • and regardless of how travellers enter the country; be it by air, train, bus/coach or car.

You are not allowed to enter the Netherlands without this completed declaration. A staff member from the airline, for example, must deny you access and you may be fined.

The form asks for information such as which very high-risk area you have been in, when you will be arriving in the Netherlands and what address you will be staying at while self-quarantining. The information you provide may be subject to checks.

Complete the declaration form

The quarantine declaration form works as follows

  • If you travel to the Netherlands from or by way of a very high-risk area, you must complete a quarantine declaration form before entering the country.

  • Once you have completed the form you will receive digital confirmation of this. Save this confirmation.

  • Have it somewhere accessible during your journey so you can show it when asked.

  • Once you arrive in the Netherlands, you must self-quarantine immediately, unless you meet the conditions for exemption.

  • The quarantine declaration is required for all means of transport.
